Housing Support Case Management
Our team of Supportworks Case Managers work with over 100 area households who are currently and formerly experiencing homelessness to help them find and/or maintain stable housing. For many of the families and individuals with whom we work, our case management model includes weekly home visits to ensure that clients are setting and meeting goals to address the challenges that may have led to homelessness.
The Representative Payee Program provides financial management by serving as an intermediary between those receiving Social Security disability payments and their benefits. The program ensures that rent and basic living expenses are paid before spending money is disbursed to clients, which keeps participants on budget and in good financial standing, thereby preventing future threat of homelessness.

Great River Terrace & The Chalet
Great River Terrace & The Chalet are permanent supportive housing communities in North and West Brattleboro respectively—offering permanent housing and support provided by Groundworks. These apartments are owned and managed by Windham & Windsor Housing Trust with on-site case management and other supports offered by Groundworks Collaborative staff in a model proven to end homelessness.
