The purpose of the Groundworks Outreach Team
The Brattleboro area currently shelters a significant number of individuals within the State of Vermont’s General Assistance emergency motel housing program. In preparation for the impending wind down of the program, as well as 100+ households potentially exiting into unsheltered homelessness, we knew that expanding Groundworks’ service model and providing targeted service coordination would be the next critical step.
What does our team do?
Outreach. Engagement. Referral.
We engage directly with people who may otherwise face significant barriers in navigating systems of support. Our individualized approach includes but is not limited to, assistance with housing, specialized health needs, obtaining identification, health insurance, primary care, etc. We also provide community-based support in regard to meeting basic needs.
The team’s presence in the community will bring us into direct contact with people living unsheltered in Brattleboro, helping us to immediately begin building relationships and creating service plans.
Who is on our team?
This team is a collaboration of Groundworks Collaborative, the Brattleboro Retreat, and Brattleboro Memorial Hospital. Our team includes a housing case manager, social worker, two community-based Advocates, and a part-time health care professional.
